The scheduling fee is NON-REFUNDABLE

• A scheduling fee is required for all New & Existing clients to schedule an office visit or surgery. Please do not pay until you have scheduled an appointment and have been instructed to pay per staff member.
• If you need to reschedule or cancel, please notify us 24 hours in advance of the scheduled appointment time, and the prepayment will remain as a credit on the client’s account
• If you cancel without sufficient notice or simply fail to show up for the appointment, the advance payment is forfeited, and there will be no credit to the client’s account.
